Aerostich jacket and pants, full-faced helmet, boots, and deerhide gloves. EVERY time I ride. (Sometimes I add a Gerbings heated jacket and a balaclava, depending.)
Oh yeah, and if I'm going faster than 50mph for more than a couple of miles, I wear earplugs. I lost significant hearing from not doing that for too many years; helmets are NOISY at speed, especially on a motorcycle with minimal fairing.
